Applications may take 10-15 business days to process enclose the following documents:
- Completed Persons with Disabilities Parking Placard and/or License Plate (VTR-214)
- Original prescription from physician OR notarized physician’s signature on the back of form VTR-214
- Valid TX DL or ID, unless active duty US Military or non resident seeking treatment in Texas
- If applying for temporary disability placards please include a check or money order made payable to Larry Gaddes, CTA
- Payments made by check may be converted to an electronic funds transfer.
