WCEMS Community Health Paramedics provide non-emergency care to the citizens of Williamson County. These Paramedics receive specialized training as a community health worker as well as advanced paramedicine training.  The goal of the group is to connect all Williamson County citizens to primary health services, thereby decreasing their reliance on 911 for primary care. In addition, the team provides specialized assistance for medically complicated patients in their homes in order to reduce preventable ambulance transports.  The CHP does not take referrals from the public.

 
What Does The CHP Do?

  • Medication management

  • Assistance obtaining primary and specialty care services for chronic medical conditions

  • Patient education

  • Insurance navigation referrals

  • Mental and behavioral health referrals

  • Placement assistance for rehab or long-term care

  • Home safety inspections and referral for modifications

  • Treatment of some conditions at home

  • Assistance in obtaining transportation

  • Food and utility assistance referrals

  • The CHP does not take referrals from the public
