Diabetes Health Plan Health Actions.  This outlines the minimum care requirements to stay enrolled in the program.

Diabetes Health Plan Prescription Drug List including medications available at no cost under this program.  To view a list of the no cost medications and supplies visit https://uhc.healthinsight.com/Wilco.  If your medication is not listed, than you will have the standard pharmacy benefits shown below.  If you wish to change to one of the no cost medications, you will need to discuss those options with your doctor.

Standard Pharmacy Benefits:
$50 Individual Deductible per Benefit Plan Year (Benefit Plan Year: November 1^st – October 31^st)
Retail: 30% up to $100 maximum per prescription
Mail-Order: $20 copayment for generic, $70 copayment for brand
